The Transitions Program
Transitions is a gentle support system provided in the comfort of your home, nursing home or rehab center - wherever you are at your time of need. This program designed to help anyone who is being challenged by a life-threatening illness, with a prognosis of one year or less. We offer trained staff and volunteers who provide invaluable support to clients and families.
How are Services Provided?
The Transitions Coordinator meets with families to offer assistance and to answer questions. Following the visit, the coordinator will make referrals to organizations within the community that can provide needed services, and will assign a volunteer to help if needed or desired.

- Understand additional resources in our community
- We can also help you qualify for Home Health Care or Hospice Care.

Why do we use volunteers?

How much does it cost?
Because the Transitions services consist mainly of a network of volunteers, there is no charge to families.
Who can refer to Transitions?
Anyone can call the Transitions Coordinator to inquire at 541-447-2510. The Coordinator will contact the individual's physician to make sure he or she thinks the Transitions support is appropriate.
