Diabetes Services

St. Charles Diabetes Services recognizes that when a patient understands the various aspects of diabetes, that knowledge leads to better diabetes management and control. Because we know that living with diabetes impacts more than just the patient, family members are encouraged to join in the educational process.

Comprehensive Diabetes Self-Management Program

Group Classes along with individual instruction sessions

This program begins with one-on-one pre-class assessment and training by Registered Nurse (RN) and Registered Dietitian (RD), plus post-class sessions includes follow-up to measure patient outcomes and behavioral goals.  Sessions include:

  • Diabetes Overview 
  • Nutrition
  • Exercise 
  • Medications
  • Blood glucose monitoring 
  • Personal health care
  • Acute/Chronic complications 
  • Psychosocial adjustment
  • Self-management 
  • Support & resources

This comprehensive program is offered once or twice a month at both sites.

Gestational Diabetes Management/Pregnancy

Individual instruction with RN & RD sessions - Sessions include:

  • Gestational or pregnancy diabetes overview
  • Blood glucose monitoring
  • Nutrition
  • Exercise 
  • If prescribed: insulin administration

Single Topics

Individual instruction with RN &/or RD

  • Understanding DM/Self BG monitoring
  • Nutrition management for diabetes with RD
  • Insulin administration, pattern management
  • Oral medications
  • Pediatric diabetes management - with RN & RD

Please note: Education noted above involves fees for service and usually a physician order. We recommend that you check with your insurance company regarding coverage issues.

Educational Support Groups

Free & open to the public

Adults- Group meetings of the diabetes educational support group offered at St. Charles inform participants about specific aspects of diabetes and provide an opportunity for the sharing of coping techniques and feelings. They are generally offered every other month and are advertised in the local newspapers as to date, time and location. Contact Karen (541) 318-4986.

Children & Families This is a quarterly support group that features activities and discussions that help support families and children coping with Pediatric Diabetes. Groups are coordinated by a parent of a Type 1 patient. Contact: Rita (541) 526-6690.

Diabetes/Pre-Diabetes Exercise Class

Referral by physician is required for this class. Fee is based service and a Physical Therapist will be your instructor. This class meets twice a week for a total of 12 weeks. Contact: (541) 388-7710

Cardiac Wellness Exercise Program

Community may self refer to this program which meets three times a week. Blood glucose & blood pressure monitored throughout program. Cost is $30-45 month.  Contact: (541) 617-2696.
